Housing Supports Program

Are you homeless or at risk of losing your housing? We can help you find stability and connect you with health care services that support your overall wellness.

Qualifications

• You are enrolled in NJ Family Care
  • Your health plan must be through one of these Managed Care Organizations (MCOs):
    • Horizon NJ Health, Aetna, WellPoint, United Healthcare, or Fidelis
• You must be a resident of Middlesex County

Services Provided

Finding housing: Receive assistance with housing applications and referrals to additional housing resources.
(Ex. Security deposit assistance, housing referrals)

Keeping your housing: Case management supports to help members maintain housing
(Ex. Job readiness, referrals, literacy classes)

Move-in support: We can provide assistance to help you set up your new home
(Ex. Assistance to purchase household items)
