Housing Supports Program

Are you homeless or at risk of losing your housing? We can help you find stability and connect you with health care services that support your overall wellness.

Qualifications

• You are enrolled in NJ Family Care
  • Your health plan must be through one of these Managed Care Organizations (MCOs):
    • Horizon, Aetna, WellPoint, United Healthcare, or Fidelis
• You are homeless or at risk of becoming homeless
  • You must have at least 2 social risk factors (e.g., currently homeless, at risk of homelessness, unstable housing)
• You have at least one health condition
  • This includes a chronic physical health condition or a mental health condition
• You must be a resident of Middlesex County

Services Provided

Finding housing: We help you search for and secure a place to live before you move in
(Ex. Security deposit)

Keeping your housing: Case management supports to help members maintain housing
(Ex. Rental assistance)

Move-in support: We can provide financial assistance to help you set up your new home
(Ex. Financial assistance to purchase household items)
