Housing Supports Program

Need support with housing? We can help you find stability and connect you with health care services that improve your overall wellness.

Qualifications

• MCO enrolled
  • Must be a recipient of NJ Family Care with one of the following MCO's
    • Horizon, Aetna, WellPoint, United Healthcare, Fidelis
• At least 1 clinical risk factor
  • (e.g., chronic health condition, mental health condition)
• At least 2 social risk factors
  • (e.g., homeless, at risk of homelessness)

Services Provided

Pre-tenancy services: Case management supports to help you find housing.
Tenancy services: Case management supports to help members maintain housing.
Move-in supports: Payment to support the setup of new housing or a move.
