HEALTHIER PERTH AMBOY MISSION:
To use the newly developed comprehensive Community Health Improvement Plan (CHIP) by Hackensack Meridian Healthcare – Raritan Bay Medical Center, to meet the healthcare needs of the community and create a healthier community where people can live, work and play.
Healthier Perth Amboy is a consortium of more than 100 entities with approximately 30 individuals and organizations working together to support healthy eating, active living, and overall wellness. Through evidence-based models to advance environmental and policy changes, and collaboration, our collective impact ensures that we can make a bigger impact by working together.
The five organizations are the Raritan Bay Area YMCA, Community Child Care Solutions, Puerto Rican Association for Human Development, Jewish Renaissance Foundation, and Raritan Bay Medical Center.
- Raritan Bay Area YMCA was officially chartered by the State of New Jersey in 1912. The YMCA is dedicated to building strong kids, strong families, and strong communities through programs that develop the values of caring, honesty, respect, faith, and responsibility. Serving all people regardless of age, religion, sex, or financial means, the YMCA is committed to developing a healthy mind, body, and spirit. The organization’s areas of focus are in youth development, healthy living and social responsibility. The Raritan Bay Area YMCA is committed to providing life-changing services to all health seekers who desire to participate.
- Community Child Care Solutions, led by the social teachings of the Church, believes that all children deserve the opportunity to develop to their fullest potential. They are dedicated to providing the highest quality of services to children, their families, and child care professionals through education, advocacy, referrals, and access to financial assistance. Community Child Care Solutions is the child care resource for families, child care professionals and the community in Somerset and Middlesex counties. They help parents find child care, support families to raise healthy children, improve the quality of child care, bridge child care and education, build the supply of child care, document child care needs and trends, engage new partners and tell the child care story.
- Puerto Rican Association for Human Development, Inc. (PRAHD) has grown from an anti-delinquency programming within Perth Amboy into a thriving comprehensive human services agency that has served thousands of New Jersey residents throughout Middlesex, Union and Hudson counties over forty years. PRAHD operates 10 programs across a wide spectrum of unmet human service needs which target education, housing, benefits access, youth development, preventative health, respite home care for the developmentally disabled, and senior services. PRAHD is dedicated to “developing human potential and promoting self-sufficiency through programs designed to improve the social, economic, health and educational status of the community in a culturally sensitive environment”. PRAHD is an active community provider and steward that constantly campaigns for the rights, dignity, and betterment of impoverished individuals and communities.
- Jewish Renaissance Foundation (JRF) was established in 1995 as a faith-based, nonprofit corporation with central offices in Perth Amboy, New Jersey. Serving more than 2,200 people in Middlesex County each year, they are a leader in providing community coordination, emergency services, education, food and nutrition, family development, and employment and training. The JRF strives to overcome poverty and hopelessness throughout Middlesex County. The JRF helps people in low-income households attain economic self-sufficiency. They provide emergency services to meet immediate needs, while fostering individual growth and transition to household stability. Job training, child care, transportation assistance and other programs are keystones to their mission of promoting greater self-sufficiency.
- Raritan Bay Medical Center, a non-profit health care organization, is committed to providing professional, compassionate and quality health care to all patients and to meeting the changing health care needs of our communities. Since 1902, Raritan Bay Medical Center has been a provider of quality compassionate care to the men, women and children in Central New Jersey. Today, they continue that high level of care, offering a total of 501 beds at our Perth Amboy and Old Bridge facilities. A medical staff of more than 500 physicians and specialists and a nationally-recognized nursing and ancillary staff of 1,700 provide medical-surgical, maternity, pediatric, general and critical care, stroke, behavioral health and emergency services. Their vision is to be the best health system in New Jersey. Their values are honesty, integrity, respect, compassion, balance and diversity. Their promise is to provide the best health care experience.
